Wednesday Ephemera: Pampell's forms

According to Wikipedia, "Ephemera is transitory written and printed matter not intended to be retained or preserved. The word derives from the Greek, meaning things lasting no more than a day."  A lot of the items in my collection of Kerrville and Kerr County historic items fit this description.
Just because the items were never meant to be kept doesn't mean they don't have historic value: you can often learn a lot from them.  Take these forms from Pampell's, a label and a tag, probably from the middle of the last century.  There are some surprises on them.
